Steve Cassabaum, MSPT, DPT

Steve is a Physical Therapist and the President of 21st Century Rehab. Steve is the Director of Therapy Services at Story County Medical Center. He is a graduate of Washington University, St. Louis, MO with a Bachelor of Science, a Master's degree, and Doctorate in physical therapy. Steve’s undergraduate degree is a Bachelor of Science in Liberal Arts from Wartburg College, Waverly, Iowa. Steve is certified in Isernhagen Work Systems’ Work Injury Management Programs and is a member of American Physical Therapy Association, Private Practice Section of APTA, Nevada Rotary Club and the Nevada Chamber of Commerce. He is a part of the Nevada GRIP Mentoring Program, and provides sports therapy to Nevada High School and Colo-NESCO High School athletics.

Jason Horras, MPT

Jason is a Physical Therapist and Vice President of 21st Century Rehab. He is also the Director of Occupational Health Services. He is a graduate of University of Iowa with a Master’s in Physical Therapy and a Bachelor of Science in Zoology from Iowa State University, Ames, Iowa. Jason is certified in Isernhagen Work Systems’ Work Injury Management Programs, is a certified occupational hearing conservationist and is also certified in DOT drug collection and testing. He is a member of American Physical Therapy Association. He also provides sports therapy to Nevada High School and Colo-NESCO High School athletics.

Monica Atherton, MPT

Monica is a Physical Therapist and provides therapy services at Perry. She is a graduate of the Mayo School of Health Sciences with a masters in Physical Therapy and a Bachelor of Science in Exercise and Movement Science from University of Oregon. She is a member of the American Physical Therapy Association. Monica volunteers for Des Moines University Significant Other Group as and advisor and is active with the Lutheran Church of Hope MOPS group service projects and serves dinner at the Catholic Worker House during the summer.

Lindsay Bierbaum, DPT

Lindsay is a physical therapist at Story County Medical Center in Nevada. Lindsay is a graduate of the University of Iowa with a Doctorate of Physical Therapy degree. She received her Bachelor of Science in Exercise Sports Science from Iowa State University. Lindsay is a member of the American Physical Therapy Association. She also belongs to the Nevada Jaycees and serves on the Communication Board for Story County Medical Center.

Jill Buesch, DPT, CSCS

Jill is a Physical Therapist and Clinic Director of Madird. Jill is a graduate of the University of Iowa with a Doctorate of Physical Therapy. She received her Bachelor of Science in Exercise Science from Iowa State University. Jill is a member of the IPTA, APTA and the Women's Health Section of the APTA. She specializes in Women's Health. She is very active with the Prairie Ridge Church in Ankeny.

Kevin Bultena, MSPT

Kevin is a Physical Therapist and the Clinic Director of Therapy Services at Hamilton Hospital. He is a graduate of Washington University, St. Louis, MO with a Master’s in Physical Therapy and a Bachelor of Science in Exercise Science from Iowa State University, Ames, Iowa. Kevin is a member of the Webster City Rotary Club and the Asbury Methodist Church.

Matt DeWall, MPT, CSCS

Matt is a Physical Therapist and Clinic Director of Indianola Physical Therapy. He is a graduate of University of Iowa with a Master’s in Physical Therapy and a Bachelor of Arts in Biology from Simpson College, Indianola, Iowa. Matt is certified in DOT drug collection and testing and is a member of American Physical Therapy Association (Orthopedic section), Iowa Physical Therapy Association, National Strength and Conditioning Association. Matt serves as board member for the Indianola Basketball Association and a little league baseball and basketball coach. He is very active in St. Thomas Aquinas Catholic Church serving on the parish council and the Knights of Columbus.

Kristin Englels, DPT

Krisitn provides physical therapy services at Franklin General Hospital in Hampton. Kristin is a graduate of Washington University in St. Louis with a Doctorate of Physical Therapy degree. She received her Bachelors of Science Degree from Waldorf College with a Major in Wellness and a Minor in Psychology. She is a member of the American Physical Therapy Association. She provides Sports Therapy and Volunteers as a softball coach at Hampton Dumont High School. Kristin is active in the Wesley United Methodist Church.

Ashley Friday, DPT, ATC/L

Ashley is a Physical Therapist and Athletic Trainer at Greene County Medical Center. She received her Doctorate of Physical Therapy from the University of Colorado Health Sciences Center in Denver, Colorado and her Bachelor of Arts from Simpson College in Indianola, Iowa. Ashley is a member of the American Physical Therapy Association and the National Athletic Training Association. She serves on the Public Health Advisory Committee and the Service Pillar at Greene County Medical Center. She is an active member of the First United Methodist Church in Jefferson, Iowa.

Jessica Hanson, PTA

Jessica is a Physical Therapist Assistant at Story County Medical Center. She is a graduate of North Iowa Area Community College with an Associate in Applied Science in the Physical Therapist Assistant program. She is member of the Americal Physical Therapy Association and the Iowa Physical Therapy Association. Jessica is active in the Stanhope Parish Church.

Matt Johnson, DPT

Matt is a physical therapist at Indianola and Altoona. Matt is a graduate of the University of Iowa with a Doctor of Physical Therapy and has an Bachelor of Science with distinction in Psychology from Iowa State University. He is the 2008 winner of the Walter R. Ingram Award for Superior Achievement in Medical Neuroscience. Matt is a member of the APTA and IPTA.

Sarah Kilbourn,OTR/L

Sarah is an Occupational Therapist at Greene County Medical Center. She received her Bachelor of Science from St. Ambrose University. Sarah is a member of the American Occupational Therapy Association.

Chad Kuehl, MPT

Chad is a Physical Therapist at Dallas County Hospital. Chad is a graduate of Des Moines University with a Master’s in Physical Therapy and Bachelor of Science from Northwest Missouri State University, Maryville, MO. He is a member of the American Physical Therapy Association and Iowa Physical Therapy Association.

Michelle Linz, MAOT, OTR/L

Michelle is an Occupational Therapist at Dallas County Hospital, Story County Medical Center and Hamilton Hospital. Michelle also provides occupational therapy services to a number of the 21st Century Rehab outpatient clinics. Michelle is a graduate of the College of St. Catherine, St. Paul, MN, with a Master’s degree in Occupational Therapy. She received her Bachelor of Arts of Phsycology and Exercise Schience from Concordia College in Moorhead, Minnesota. Michelle is involved in St. Thomas Aquanis Church in Ames.

Jamie Luedtke, OTR/L

Jamie is an Occupational Therapist at Dallas County Hospital. She received her Master of Occupational Therapy from Rockhurst University in Kansas City, Missouri and her Bachelor of Science in Health Science from Truman State University in Kirksville, Missouri. Jamie is a member of the Iowa Occupational Therapy Association and The National Occutpational Therapy Association. She serves on the Service Pillar at Dallas County Hospital.

Clint Lutterman, DPT, OCS

Clint is a Physical Therapist and the Clinic Director of Therapy Services at Dallas County Hospital. He is a graduate of Creighton University of Omaha, NE with a Doctorate in Physical Therapy and a Bachelor of Arts in Biology from Northwestern College in Orange City, IA. He is an orthopedic clinical specialist and a member of American Physical Therapy Association (Sports Physical Therapy section) and the Perry Rotary Club. Clint provides sports therapy to Perry High School athletics.

Amanda Marine, MA, SLP

Amanda, SLP, is a speech-language pathologist. She provides speech therapy services at Story County Medical Center in Nevada, Dallas County Hospital in Perry and Hamilton Hospital in Webster City. Amanda is a graduate of the University of Northern Iowa with a Masters of Speech-Language Pathology degree. She received her Bachelor’s in Communicative Disorders from UNI as well.

Tara Marler, COTA

Tara is a certified occupational therapy assistant at Story County Medical Center. Tara is a graduate of Kirkwood Community College in Cedar Rapids, IA with an Associates of Applied Science Degree in the Occupational Therapy Assistant Program. She is also a member of the American Occupational Therapy Association, Brain Injury Association of Iowa, National Multiple Sclerosis Society and Brain Injury Association of America. Tara also provides occupational therapy services to Hamilton Hospital in Webster City and Altoona on an as needed basis.

Deb Morton, MPT, ATC/L

Deb Morton is a Physical Therapist and Athletic Trainer and Director of Rehab at Greene County Medical Center. She received her Masters of Physical Therapy from Southwest Baptist University and her Bachelor of Arts in Athletic Training from Graceland University. Deb is a member of the American Physical Therapy Association. She provides Athletic Training to Greene County Schools and volunteers as a coach for Greene County Youth Sports. She is a member of the First United Methodist Church in Jefferson, Iowa and volunteers on the Wellnes Committee at Greene County Medical Center.

Courtney Murphy, PTA

Courtney Murphy is a Physical Therapy Assistant at Franklin General Hospital. She received her Associate of Applied Science Degree in Physical Therapy from North Iowa Area Community College. Courtney is an umpire for Hampton Dumont softball.

Lisa Nerem, PTA

Lisa is a Physical Therapist Assistant. She is a graduate of Mercy College of Health Sciences with an Associate in Applied Science in the Physical Therapist Assistant program. She is a member of the American Physical Therapy Association.

Austin Ollinger, PTA

Austin is a Physical Therapy Assistant at Knoxville Hospitals and Clinics and at the Carlisle location. He received his Bachelor of Science in Health and Human Performance from Iowa State University and is a graduate of Mercy College of Health Sciences with an Associate in Applied Science in the Physical Therapist Assistant program. Austin is active in the Lutheran Church of Hope and volunteers for the Iowa Games.

Steve Olsen, MSPT, CSCS

Steve is a Physical Therapist and Clinic Director of Altoona. Steve is a graduate of Des Moines University with a Master’s in Physical Therapy and Bachelor of Arts from Central College, Pella, Iowa in Physical Education. He is certified in Isernhagen Work Systems’ Work Injury Management Programs. Steve is also a certified occupational hearing conservationist and is certified in DOT drug collection and testing. He is a member of American Physical Therapy Association, Iowa Physical Therapy Association, National Strength and Conditioning Association, Altoona Rotary Club, Altoona Kiwanis and the Altoona Chamber of Commerce. Steve also provides sports therapy to Bondurant-Farrar High School athletics.

Paul Ouverson, DPT, CEAS

Paul is a Physical Therapist and the Clinic Director of Therapy Services at Carlisle. He is a graduate of Creighton University of Omaha, NE with a Doctorate in Physical Therapy and a Bachelor of Science in Exercise and Sports Science from Iowa State University. He is a certified ergonomic assessment specialist. Paul provides sports therapy to Carlisle High School athletics.

Rebecca Phillips, DPT

Becky is a Physical Therapist and the Director in Carlisle. She received her Doctorate of Physical Therapy, her Master of Science in Physical Therapy and her Bachelor of Exercise Science all from Saint Louis University. She is a member of the American Physical Therapy Association where she belongs to both the Orthopedic and Wound Care Sections and belongs to the Iowa Physical Therapy Association. Becky is active in Saints John and Paul Catholic Church in Altoona where she is an interfaith hospitality host, works with the youth group and is a lector.

Nicole Pontier, ATC, LAT

Nicole is a certified and licensed athletic trainer. She is a graduate of Coe College with a Bachelor of Arts in Athletic Training. She is a member of the National Athletic Trainer’s Association. She is the Patient Representative for the Carlisle location and provides athletic training to Carlisle Schools.

Tracy Porter, MSPT

Tracy is a Physical Therapist at Madrid. She also provides therapy services at Story County Medical Center Nevada. Tracy is a graduate of Des Moines University with a Master’s in Physical Therapy and a Bachelor of Arts in Biology from University of Northern Iowa, Cedar Falls, Iowa. She is a group fitness instructor for Ames Racquet & Fitness and is a member of Memorial Lutheran Church in Ames where she participates in choir and is a Sunday School teacher. Tracy is involved with One Heart Equestrian Therapy and is a member of Beta Sigma Phi.

Jamie Rabe, DPT, ATC

Jamie is a physical therapist and certified athletic trainer. She provides physical therapy services at Story County Medical Center in Nevada. Jamie is a graduate of the University of Iowa with a Doctorate of Physical Therapy degree. She received her Bachelor of Science Degree from Iowa State University. She is a member of the American Physical Therapy Association, the Iowa Physical Therapy Association and the National Athletic Trainer's Association. Jamie is a member of the Collegiate United Methodist Church and Wesley Foundation in Ames. She is an Iowa Special Olympics Volunteer.

Libby Rasmussen, DPT

Libby is a Physical Therapist at Van Diest Medical Center in Webster City. She received her Doctor of Physical Therapy from Des Moines University, her Bachelor of Arts in Kinesiology from Iowa State University and her Associates Degree from Iowa Central Community College. Libby is a member of the American Physical Therapy Association and the Iowa Physical Therapy Association. She is active in St. Paul Lutheran Church in Webster City.

Deonne Reed, PTA

Deonne is a Physical Therapy Assistant at Greene County Medical Center. She received her Bachelor of Arts from Buena Vista University in Storm Lake, Iowa and her Associate in Applied Science in Physical Therapy form Western Iowa Tech. Deonne volunteers on the ServiShare Safety Committee in support of improving overall hosptial safety in Iowa. She also volunteers on the Greene County Medical Center Safety Committee and the Employee Health Committee. Deonne is a Board Member for the William patien Public Library and is active in the Salem United Methodist Church in Paton.

Kate Reiter, MAOT, OTR/L

Kate is an Occupational Therapist at Franklin General Hospital in Hampton. Kate is a graduate of the College of St. Catherine, St. Paul, MN, with a Master’s degree in Occupational Therapy and a Bachelor of Science degree from Iowa State University in Exercise and Sport Science with a minor in Health Studies. She is a member of the Americal Occupational Therapy Association and the Iowa Occupational Therapy Association. Kate is Certified Lymphedema Specialist. She serves on the Wellness Committee at Franklin General Hospital.

Nancy Hunter Schreier, OTR/L

Nancy is an Occupational Therapist at Story County Medical Center. She is a graduate of Iowa State University with a Bachelor of Science in Psychology and Bachelor of Science in Occupational Therapy from College of St. Catherine, St. Paul, MN. Nancy is a member of the American Occupational Therapy Association and St. Patrick’s Catholic Church.

Keri Sersland, ATC, LAT, PTA

Keri is a certified and licensed Athletic Trainer and is a graduate of University of Northern Iowa with a Bachelor of Science in Athletic Training. In May 2011, Keri graduated from the physical therapy assistant program at Mercy Health College. Keri is a member of National Athletic Training Association and the American Physical Therapy Association. Keri provides athletic coverage to Perry Schools.

Brooke Shelquist, DPT

Brooke is a Physical Therapist at Knoxville Hospitals and Clinics along with the Indianola and Carlisle locations. She received her Doctor of Physical Therapy from Des Moines University and her Bachelor of Science from the Univeristy of North Carolina at Wilmington. Brooke is very active in the soccer community around Des Moines.

Joel Stam, PTA

Joel is a Physical Therapist Assistant in Altoona and Story County Medical Center. He is a graduate of Mercy College of Health Sciences with an Associate in Applied Science in the Physical Therapist Assistant program and a Bachelor of Arts in Exercise from Dordt College. Joel is a member of the American Physical Therapy Association.

Tasha Vorm, ATC, LAT, PTA

Tasha is a certified and licensed Athletic Trainer and Physical Therapy Assitant at Story County Medical Center. Tasha is a graduate of Iowa State University with a Bachelor of Science in Exercise and Sport Science with a minor in Athletic Training. In May 2010, Tasha was a member of the first graduating class in the physical therapy assistant program at Mercy Health College. Tasha is a member of National Athletic Training Association and the Iowa Athletic Trainers' Society, the American Physical Therapy Association and the Iowa Physical Therapy Association. Tasha is a Certified Occupational Health Hearing Conservationist, a Certified DOT drug Speciman Collecter, a Performance Enhancement Specialist and is and American Red Cross CPR instructor. She volunteers as a second grade religion teacher at St. Patrick’s Catholic Church and is the secretary for St. Patrick’s Life and Growth Committee. She is volunteer coach for Nevada Parks and Rec and provides sports medicine services for Nevada High School Athletics.

Teresa White, PTA

Teresa is a Physical Therapist Assistant at Dallas County Hospital. She is a graduate of Indian Hills Community College, Ottumwa, IA with an Associate in Applied Science in the Physical Therapist Assistant program. She serves on the Dallas County Hospital Public Relations Committee (P.R.I.D.E.) and C.A.R.E. Subcommittee, is part of Dallas County Hospital Volunteer Auxiliary, Ladies Nite Out (provides women's health education seminars), and Dallas County Hospital Standards Committee.

Susan Woodle, DPT

Susan is a Physical Therapist and the Director of Rehab at Knoxville Hospitals and Clinics. She received her Doctor of Physical Therapy from St. Ambrose University, Masters of Science in Physical Therapy from Texas Women's Universtiy, Masters in Education in Guidance and Counseling from the University of Texas-Pan Am, Bachelor of Science in Biology and her Bachelor of Arts in Psychology from Graceland Universtiy. Susan is a member of the American Physical Therapy Association where she belongs to the Orthopedic Section and she is a member of the Iowa Physical Therapy Association. She is a Certified Clinical Instructor and specializes in chronic pain management, fibromyalgia and arthritis. Susan is very active in her community including the First United Methodist Church, Habitat for Hummanity, Hospice, and PTA.

Keith Wrich, DPT, ATC, LAT

Keith is a Physical Therapist and Athletic Trainer at Franklin General Hospital in Hampton. He received his Doctor of Physical Therapy from Des Moines Univeristy and his Bachelor of Arts in Exercise Science from Central College. Keith is a member of the American Physical Therapy Association and the Iowa Physical Therapy Association. He provides sports medicine coverage to Hampton-Dumont Schools. He is actively involved in the Faith Baptist Church in Hampton.

Sara Zorn, OTR/L

Sara is a licensed occupational therapist at Hamilton Hospital dba Van Diest Medical Center and Franklin General Hospital. She is a graduate of Concordia University with her Masters of Occupational Therapy and her Bachelors in Rehab Science. She is a graduate of Western Iowa Tech Community College, Sioux City, Iowa with a degree in Occupational Therapy Assistant. Outside of work, Sarah is a volunteer EMT with the Williams Ambulance Service and also enjoys spending time with her family. She volunteers for the Board of Education and as a T-ball coach. She is an instructor in CPR and a member of the American Occupational Therapy Association as well as a member of the local Iowa Occupational Therapy Association.
