Steve Cassabaum, MSPT, DPT

Steve is a Physical Therapist and the President of 21st Century Rehab. Steve is the Director of Therapy Services at Story County Medical Center. He is a graduate of Washington University, St. Louis, MO with a Bachelor of Science, a Master's degree, and Doctorate in Physical Therapy. Steve’s undergraduate degree is a Bachelor of Science in Liberal Arts from Wartburg College, Waverly, Iowa. Steve is certified in Work Well Injury Management Programs, Dry Needling and is a member of American Physical Therapy Association, Private Practice Section of APTA, member and immediate Past President of the Iowa Physical Therapy Association, Nevada Rotary Club and the Nevada Chamber of Commerce. He provides sports therapy to Nevada High School athletics.

Jason Horras, MPT, DPT

Jason is a Physical Therapist and Vice President of 21st Century Rehab. He is also the Director of Occupational Health Services. He is a graduate of Washington University, St. Louis, MO with a Doctor in Physical Therapy He is a graduate of University of Iowa with a Master’s in Physical Therapy and a Bachelor of Science in Zoology from Iowa State University, Ames, Iowa. Jason is certified in Isernhagen Work Systems’ Work Injury Management Programs, is a certified occupational hearing conservationist and is also certified in DOT drug collection and testing. He is a member of American Physical Therapy Association and Iowa Physical Therapy Association.

Ashley Beekman, PTA, ATC, LAT

Ashley is a Certified and Licensed Athletic Trainer and Physical Therapy Assistant at Greene County Medical Center in Jefferson, IA. Ashley is a graduate of Mercy College of Health Sciences where she received her Associate of Science in the Physical Therapist Assistant program. Ashley received her Bachelor of Science in Athletic Training from Central College, Pella, IA. She is a member of Iowa Physical Therapy Association and American Physical Therapy Association. Ashley provides sports medicine services for Greene County Schools Athletics. She also volunteers on the Wellness Committee at Greene County Medical Center.

Jessica Bell, PTA

Jessica is a Physical Therapist Assistant at Story County Medical Center. She is a graduate of North Iowa Area Community College with an Associate in Applied Science in the Physical Therapist Assistant program. She is member of the American Physical Therapy Association and the Iowa Physical Therapy Association. Jessica is active in the Stanhope Parish Church.

Lindsay Bierbaum, DPT

Lindsay is a Physical Therapist and Clinic Director at Story County Medical Center in Nevada. Lindsay is a graduate of the University of Iowa with a Doctorate of Physical Therapy degree. She received her Bachelor of Science in Exercise Sports Science from Iowa State University. Lindsay is a member of the American Physical Therapy Association and the Iowa Physical Therapy Association. She also belongs to the Nevada Jaycees and serves on the Communication Board for Story County Medical Center.

Sara Boyce, PTA

Sara is a Physical Therapist Assistant at Knoxville Hospitals and Clinics. She is a graduate of Indian Hills Community College with an Associate in Applied Science in the Physical Therapist Assistant program. She is member of the American Physical Therapy Association and the Iowa Physical Therapy Association. Sara is active at Grace Baptist Church and volunteers on the Wellness Committee at Knoxville Hospitals and Clinics.

Kelci Brannen, MS, CCC-SLP

Kelci is a Speech Language Pathologist at Story County Medical Center in Nevada, Dallas County Hospital in Perry, Van Diest Medical Center in Webster City, and Franklin General Hospital in Hampton. Kelci is a graduate of St. Ambrose University with a Master of Science in Speech-Language Pathology. She received her Bachelor of Arts in Psychology from St. Ambrose University. Kelci is a member of the American Speech and Hearing Association and the Iowa Speech and Hearing Association. She has her Certificate of Clinical Competence.

Jill Buesch, DPT, CSCS

Jill is a Physical Therapist and Clinic Director of Madird. Jill is a graduate of the University of Iowa with a Doctorate of Physical Therapy. She received her Bachelor of Science in Exercise Science from Iowa State University. Jill is a member of the Iowa Physical Therapy Association and the American Physical Therapy Association and the Women's Health Section of the APTA. She specializes in Women's Health. She is active with the Prairie Ridge Church in Ankeny.

Kevin Bultena, DPT

Kevin is a Physical Therapist and the Clinic Director of Therapy Services at Van Diest Medical Center. He is a graduate of Washington University, St. Louis, MO with a Master’s in Physical Therapy and a Bachelor of Science in Exercise Science from Iowa State University, Ames, Iowa. Kevin is a member of the American Physical Therapy Association and the Iowa Physical Therapy Association. He is also a member of the Webster City Rotary Club and the Asbury Methodist Church.

Matt DeWall, DPT, CSCS

Matt is a Physical Therapist and Clinic Director of Indianola Physical Therapy. He is a graduate of University of Iowa with a Master’s in Physical Therapy and a Bachelor of Arts in Biology from Simpson College, Indianola, Iowa. Matt is certified in DOT drug collection and testing, Dry Needling and is a member of American Physical Therapy Association (Orthopedic section), Iowa Physical Therapy Association, National Strength and Conditioning Association.  Matt serves as board member for the Indianola Basketball Association and a little league baseball and basketball coach. He is very active in St. Thomas Aquinas Catholic Church serving on the parish council and the Knights of Columbus.

Kristin Engels, DPT, CLT

Kristin is a Physical Therapist and Clinic Director of Therapy Services at Franklin General Hospital in Hampton. Kristin is a graduate of Washington University in St. Louis with a Doctorate of Physical Therapy degree. She received her Bachelors of Science Degree from Waldorf College with a Major in Wellness and a Minor in Psychology. Kristin is also Lymphedema certified. She is a member of the American Physical Therapy Association and the Iowa Physical Therapy Association. She provides Sports Therapy and Volunteers as a softball coach at Hampton Dumont High School. Kristin is active in the Wesley United Methodist Church.

Keri Fye, PTA, ATC, LAT

Keri is a Certified and Licensed Athletic Trainer and is a graduate of University of Northern Iowa with a Bachelor of Science in Athletic Training. In May 2011, Keri graduated from the Physical Therapist Assistant program at Mercy College of Health Sciences. Keri is a member of National Athletic Training Association and the American Physical Therapy Association. Keri provides athletic coverage to Perry Schools.

Micah J. Hayek, DPT

Micah is a Physical Therapist at Story County Medical Center in Nevada. Micah is a graduate of the University of Iowa with a Doctorate of Physical Therapy degree. He received his Bachelor of Science of Kinesiology and Health from Iowa State University. Micah is a member of the American Physical Therapy Association and the Iowa Physical Therapy Association. Micah is working toward becoming a Certified Strength and Conditioning Specialist. He also volunteers as a Coordinator for Wheels for the World, helping to collect used medical equipment like wheelchairs, walkers, canes and crutches to be refurbished and sent to meet the needs in foreign countries.

Eric Holton, DPT

Eric is a Physical Therapist in Carlisle. Eric is a graduate of Des Moines University with a Doctorate of Physical Therapy degree. He received his Bachelor of Science in Biology from Buena Vista University. Eric is a member of the American Physical Therapy Association and the Iowa Physical Therapy Association.

Samantha Imholte, DPT

Sam is a Physical Therapist at Wayne County Hospital in Corydon, IA. Sam is a graduate of Creighton University with a Doctorate of Physical Therapy degree. Samanha is a member of the American Physical Therapy Association and the Iowa Physical Therapy Association.

 

Matt Johnson, DPT

Matt is a Physical Therapist in Indianola. Matt is a graduate of the University of Iowa with a Doctor of Physical Therapy and has an Bachelor of Science with distinction in Psychology from Iowa State University. He is the 2008 winner of the Walter R. Ingram Award for Superior Achievement in Medical Neuroscience. Matt is a member of the American Physical Therapy Association and Iowa Physical Therapy Association.

Jenna Karas, OTR/L

Jenna is an Occupational Therapist at Knoxville Hospitals & Clinics.  Jenna received her Master's in Occupational Therapy from College of Saint Mary in Omaha, NE and her Bachelor's in Rehabilitation Studies from College of Saint Mary in Omaha, NE.  Jenna is a member of the AOTA.  Jenna is originally from SW Iowa and is looking forward to being in Knoxville, IA.

Andrea Knowles, PTA

Andrea is a Physical Therapy Assistant at Wayne County Medical Center in Corydon, IA. Andrea is a graduate of Indian Hills Community College where she received her Associate of Applied Science in the Physical Therapist Assistant program. Andrea received her Bachelor of Science in Exercise Science and Human Performance & Fitness from St. Ambrose University and her Master of Science in Kinesiology from Iowa State University. She is a member of Iowa Physical Therapy Association and American Physical Therapy Association.

Chad Kuehl, MPT, DPT

Chad is a Physical Therapist at Dallas County Hospital. Chad is a graduate of Washington University with a Doctor of Physical Therapy and Des Moines University with a Master’s in Physical Therapy and Bachelor of Science from Northwest Missouri State University, Maryville, MO. He is a member of the American Physical Therapy Association and Iowa Physical Therapy Association.

Laurie Loving, DPT

Laurie is a Physical Therapist at VanDiest Hospital in Webster City.  Laurie is a graduate of Creighton University with a Doctor of Physical Therapy and Bachelor of Science in Health Science.  She is a member of the American Physical Therapy Association and Iowa Physical Therapy Association.

Clint Lutterman, DPT, OCS

Clint is a Physical Therapist and the Clinic Director of Therapy Services at Dallas County Hospital. He is a graduate of Creighton University in Omaha, NE with a Doctorate in Physical Therapy and a Bachelor of Arts in Biology from Northwestern College in Orange City, IA. He is an orthopedic clinical specialist, certified in Dry Needling and a member of American Physical Therapy Association (Sports Physical Therapy section), the Iowa Physical Therapy Association and the Perry Rotary Club. Clint provides sports therapy to Perry High School athletics.

Tara Marler, COTA

Tara is a Certified Occupational Therapy Assistant at Story County Medical Center. Tara is a graduate of Kirkwood Community College in Cedar Rapids, IA with an Associates of Applied Science Degree in the Occupational Therapy Assistant Program. She is also a member of the American Occupational Therapy Association, the Iowa Occupational Therapy Association, and National Multiple Sclerosis Society.

 

Brittany McCall, DPT, CLT

Brittany is a Physical Therapist at Story County Medical Center in Nevada, IA.  Brittany is a graduate of Des Moines University in Des Moines, IA, with a Doctorate of Physical Therapy degree.  She received her Bachelor of Science in Kinseiology from Iowa State Univeristy.  She belongs to the APTA and the IPTA and is active in the Women's Health section.  Brittany specializes her care in Women's Health and Pelvic Health, and Lymphedema.  Brittany is active in Life Is Bliss Dragon Boat Association, and Cornerstone Church of Ames.

Caitlin McDowell, MS, OTR/L

Caitlin is an Occupational Therapist at Dallas County Hospital in Perry and Greene County Hospital in Jefferson. She is a graduate of University of Iowa with a Bachelor of Science in Psychology and MS Occupational Therapy from the University of South Dakota, SD.  Caitlin is a member of the IOTA and AOTA and Brain Injury Alliance of Iowa.

Deb Morton, MPT, ATC/L

Deb Morton is a Physical Therapist, Athletic Trainer and Director of Rehab at Greene County Medical Center. She received her Masters of Physical Therapy from Southwest Baptist University and her Bachelor of Arts in Athletic Training from Graceland University. Deb is a member of the American Physical Therapy Association and the Iowa Physical Therapy Association. She provides Athletic Training to Greene County Schools and volunteers as a coach for Greene County Youth Sports. She is a member of the First United Methodist Church in Jefferson, Iowa.

Courtney Murphy, PTA

Courtney Murphy is a Physical Therapy Assistant at Franklin General Hospital. She received her Associate of Applied Science Degree in Physical Therapy from North Iowa Area Community College.

Matt Myers, MS,ATC,LAT,COCH, CSCS

Matt Myers is our Director Of Athletic Training Services. He received his Master of Science in Management from Minot State University in Minot, North Dakota and Bachelor of Science in Kinesiology and Health with an emphasis in Athletic Training from Iowa State University. He is a member of the National Athletic Trainers Association, Mid America Athletic Trainers Association, and Iowa Athletic Trainers Society. Matt is a Certified Occupational Health Hearing Conservationist.

Jessica Nelson, DPT

Jessica is a Physical Therapist in Indianola.Jessica is a graduate of Des Moines University with a Doctorate in Physical Therapy and a Bachelor of Science in Kinesiology from the University of Wisconsin Milwaukee.  Jessica is a member of the IPTA and the APTA.

Lisa Nerem, PTA

Lisa is a Physical Therapist Assistant in Madrid and Van Diest Medical Center in Webster City. She is a graduate of Mercy College of Health Sciences with an Associate of Science in the Physical Therapist Assistant program. Lisa is a certified clinical instructor for PTA students. She is a member of the American Physical Therapy Association and the Iowa Physical Therapy Association. She is an active member at Trinity Lutheran Church in Boone, IA.

Austin Ollinger, PTA

Austin is a Physical Therapist Assistant at Knoxville Hospitals and Clinics. He received his Bachelor of Science in Health and Human Performance from Iowa State University and is a graduate of Mercy College of Health Sciences with an Associate of Science in the Physical Therapist Assistant program. Austin is active in the Lutheran Church of Hope and volunteers for the Iowa Games.

Rachel Palen, DPT

Rachel is a Physical Therapist at Dallas County Hospital in Perry.  She is a graduate of University of Northern Iowa with a BS in Movement and Exercise Science.  She received her Doctor of Physical Therapy from Des Moines University.  Rachel is a member of IPTA and APTA.  She volunteers for Meals for the Heartland.

Eric Palmer, DPT

Eric is a Physical Therapist and Clinic Director at Wayne County Hospital in Corydon, IA. He received his Doctor of Physical Therapy from Des Moines University and her Bachelors in Exercise Science from University of Sioux Falls. He is a member of the American Physical Therapy Association and Iowa Physical Therapy Association. Eric is an active member of the First Baptist Church. Eric volunteers as Youth Leader for high school students, youth soccer, basketball and t-ball coach. He is also a volunteer for the Community Betterment Foundation.

Rebecca Phillips, DPT,CLT

Becky is a Physical Therapist in Carlisle, IA and at Knoxville Hospital and Clinics. She received her Doctorate of Physical Therapy, her Master of Science in Physical Therapy and her Bachelor of Exercise Science all from Saint Louis University. She is a Certified Lymphedema Therapist and provides therapy in Carlisle. She is a member of the American Physical Therapy Association where she belongs to both the Orthopedic and Wound Care Sections and belongs to the Iowa Physical Therapy Association. Becky is active in Saints John and Paul Catholic Church in Altoona where she is an interfaith hospitality host, works with the youth group and is a lector.

Justin Plum, DPT, OCS, Cert. MDT

Justin is a Physical Therapist and Director of Rehab at Knoxville Hospital & Clinics. Justin received his Doctorate of Physical Therapy from the University of Iowa. He received his Bachelor of Arts in Biology from the University of Northern Iowa. Justin is certified in Mechanical Diagnosis and Therapy (McKenzie Method) and Dry Needling. Justin is a member of the American Physical Therapy Association Sports Section and the Iowa Physical Therapy Association.

Lauren Plum, DPT, OCS

Lauren is a Physical Therapist at Wayne County Hospital in Corydon, IA. Lauren received her Doctorate of Physical Therapy from the University of Iowa. She received her Bachelor of Science in Exercise Science from Southeast Missouri State University  Lauren is certified in Dry Needling. She is a member of the American Physical Therapy Association Orthopedic Section as well as the Iowa Physical Therapy Association and is a Certified Clinical Instructor. Lauren is active in the dance community.

Jamie Rabe, DPT,CLT, ATC

Jamie is a Physical Therapist, Certified Lymphedema Specialized and a Certified Athletic Trainer. She provides physical therapy services at Dallas County Hospital in Perry, IA and fills in at Story County Medical Center when needed. Jamie is a graduate of the University of Iowa with a Doctorate of Physical Therapy degree. She received her Bachelor of Science Degree from Iowa State University. She is a member of the American Physical Therapy Association, the Iowa Physical Therapy Association and the National Athletic Trainer's Association. Jamie is a member of the Collegiate United Methodist Church and Wesley Foundation in Ames. She is an Iowa Special Olympics Volunteer.

Diana Raska, OTR/L

Diana is an Occupational Therapist at Frankling General Hospital in Hampton and VanDiest in Webster City, IA.  She is a graduate of Buena Vista University with a BA in Organizational Leadership and a Minor in Psychology.  Diana received her Masters of Occupatinoal Therapy from University of Minnesota Rochester.  Diana is currently a member of the AOTA, IOTA and MOTA.

Deonne Robey, PTA

Deonne is a Physical Therapist Assistant at Greene County Medical Center. She received her Bachelor of Arts from Buena Vista University in Storm Lake, Iowa and her Associate in Applied Science in Physical Therapy from Western Iowa Tech. Deonne is a member of the Greene County Medical Center Safety Committee,and a Board member of the Paton Churdan Community School District. Deonne also volunteers with the Early Education Pre-School planning committee and is an active member in the Salem United Methodist Church in Paton.

Nancy Hunter-Schreier, OTR/L, CLT

Nancy is an Occupational Therapist and a Certified Lymphedema Specialist at Story County Medical Center. She is a graduate of Iowa State University with a Bachelor of Science in Psychology and Bachelor of Science in Occupational Therapy from College of St. Catherine, St. Paul, MN. Nancy is a member of the American Occupational Therapy Association and St. Patrick’s Catholic Church.

Crystol Shear, PTA

Crystol is a Physical Therapist Assistant at Franklin General Hospital in Hampton. She is a graduate of North Iowa Area Community College with an Associate in Applied Science in the Physical Therapist Assistant program. Crystol is a volunteer for Joppa Outreach. Crystol is a member of the Lutheran Church of Hope and St. John's Lutheran Church.

Brooke Shelquist, DPT

Brooke is a Physical Therapist at Knoxville Hospitals and Clinics along with the Indianola and Carlisle locations. She received her Doctor of Physical Therapy from Des Moines University and her Bachelor of Science from the University of North Carolina at Wilmington. She is a member of the American Physical Therapy Association and Iowa Physical Therapy Association. Brooke is very active in the soccer community around Des Moines.

Joel Stam, PTA

Joel is a Physical Therapist Assistant in Altoona. He is a graduate of Mercy College of Health Sciences with an Associate of Science in the Physical Therapist Assistant program and a Bachelor of Arts in Exercise Science from Dordt College. Joel is a member of the American Physical Therapy Association and Iowa Physical Therapy Association. He is also a certified clinical instructor for PTA students. His passions include running and playing Ultimate Frisbee for the nationally ranked club team Des Moines Metro. Joel is an active member of Providence United Reformed Church in Des Moines.

Trina Stone, DPT

Trina is a Physical Therapist at Franklin General Hospital in Hampton, IA. She is a graduate of Des Moines University with a Doctor of Physical Therapy and received her Bachelor of Science from Iowa State University in Kinesiology and Associate of Science in Biology from North Iowa Area Community College.  Trina is certified in Dry Needling. Trina is a member of American Physical Therapy Association and Iowa Physical Therapy Association. Trina and her husband enjoy playing beach volleyball.

Natalie Theroux, DPT

Natalie is a Physical Therapist in Indianola. Natalie is a graduate of Des Moines University with a Doctor of Physical Therapy and Bachelor of Arts from Central College, Pella, Iowa in Exercise Science. She is a member of American Physical Therapy Association and Iowa Physical Therapy Association. Natalie is a National Strength and Conditioning Association Certified Personal Trainer. She is active in the softball community around Des Moines.

Tasha Vorm, PTA, ATC, LAT, COCH

Tasha is a Certified and Licensed Athletic Trainer and Physical Therapist Assistant at Story County Medical Center. Tasha is a graduate of Iowa State University with a Bachelor of Science in Exercise and Sport Science with a minor in Athletic Training. In May 2010, Tasha was a member of the first graduating class in the Physical Therapist Assistant program at Mercy College of Health Sciences. Tasha is a member of National Athletic Training Association and the Iowa Athletic Trainers' Society, the American Physical Therapy Association and the Iowa Physical Therapy Association. Tasha is a Certified Occupational Health Hearing Conservationist, a Certified DOT drug Specimen Collector, a Certified Alcohol Test Technician and an American Red Cross CPR Instructor. She volunteers as a second grade religion teacher at St. Patrick’s Catholic Church. She is volunteer coach for Nevada Parks and Rec and provides sports medicine services for Nevada High School Athletics.

Matthew Wanderscheid, DPT, ATC

Matt is a Physical Therapist in Carlisle, IA and Dallas County Hospital in Perry, IA.  He is a graduate of University of Jamestown in North Dakota with a Doctorate of Physical Therapy.  He received his Bachelor of Science in Athletic Training from South Dakota State University.  Matt is certified in Dry Needling.  Matt is currently a member of APTA, St. Elizabeth Seton Church in Carlisle and participates in the Carlisle Chamber of Commerce events.  Matt also helps cover athletic events at area high schools as needed.  He has volunteered in horseback riding therapy, Special Olympics and fitness classes for disabled children throughout his time in school.  His hobbies include kayaking, hunting, fishing and riding his bike on local trails.

Danielle Weber, DPT,CLT

Danielle is a Physical Therapist at Greene County Medical Center in Jefferson, IA. She is a graduate of Des Moines University with a Doctor of Physical Therapy and received her Bachelor of Science from Buena Vista University in Storm Lake, IA. She is a Certified Lymphedema Therapist. She has a special interest in treating lymphedema, vestibular dysfunction, back pain, orthopedic injuries, sports injuries, and post-op rehabilitation.

Teresa White, PTA

Teresa is a Physical Therapist Assistant at Dallas County Hospital. She is a graduate of Indian Hills Community College, Ottumwa, IA with an Associate in Applied Science in the Physical Therapist Assistant program. She serves on the Dallas County Hospital Public Relations Committee (P.R.I.D.E.) and C.A.R.E. Subcommittee, is part of Dallas County Hospital Volunteer Auxiliary, Ladies Nite Out (provides women's health education seminars), and Dallas County Hospital Heart Committee.

Susan Woodle, MS,PT, DPT, CLT

Susan is a Physical Therapist at Knoxville Hospitals and Clinics. She received her Doctor of Physical Therapy from St. Ambrose University, Masters of Science in Physical Therapy from Texas Women's University, Masters in Education in Guidance and Counseling from the University of Texas-Pan Am, Bachelor of Science in Biology and her Bachelor of Arts in Psychology from Graceland University. Susan is a member of the American Physical Therapy Association where she belongs to the Orthopedic Section and she is a member of the Iowa Physical Therapy Association. She is a Certified Lymphedema Therapist, Certified Clinical Instructor and specializes in chronic pain management, fibromyaliga, and arthritis. Susan is active in her community including the Celebrate Church, Habitat for Humanity, and Hospice.

Keith Wrich, DPT, ATC, LAT

Keith is a Physical Therapist and Athletic Trainer at Franklin General Hospital in Hampton. He received his Doctor of Physical Therapy from Des Moines University and his Bachelor of Arts in Exercise Science from Central College in Pella, IA. Keith is a member of the American Physical Therapy Association and the Iowa Physical Therapy Association. He provides sports medicine coverage to Hampton-Dumont Schools. He is actively involved in the Faith Baptist Church in Hampton.

Sara Zorn, MOT,OTR/L,CLT

Sara is a Licensed Occupational Therapist at Van Diest Medical Center and Franklin General Hospital. She is a graduate of Concordia University with her Masters of Occupational Therapy and her Bachelors in Rehab Science. She is a graduate of Western Iowa Tech Community College, Sioux City, Iowa with a degree in Occupational Therapy Assistant. Sara is a Certified Lymphedema Specialist. Outside of work, Sara is a volunteer EMT with the Williams Ambulance Service and also enjoys spending time with her family. She volunteers for the Board of Education and as a T-ball coach. She is an instructor in CPR and a member of the American Occupational Therapy Association as well as a member of the local Iowa Occupational Therapy Association.
