Steve Cassabaum, MSPT
Steve is a Physical Therapist and the president of 21st Century Rehab. Steve is the Director of Therapy Services at Story County Medical Center. He is a graduate of Washington University, St. Louis, MO with both a Bachelor of Science and a Master's degree in physical therapy.  Steve’s undergraduate degree is a Bachelor of Science in Liberal Arts from Wartburg College, Waverly, Iowa. Steve is certified in Isernhagen Work Systems’ Work Injury Management Programs and is a member of American Physical Therapy Association, Private Practice Section of APTA, Nevada Rotary Club and the Nevada Chamber of Commerce. He sits on the Krysilis Board of Directors, is a part of the Nevada GRIP Mentoring Program, and provides sports therapy to Nevada High School and Colo-NESCO High School athletics.

Jason Horras, MPT
Jason is a Physical Therapist and vice president of 21st Century Rehab. He is also the Director of Occupational Health Services. He is a graduate of University of Iowa with a Master’s in Physical Therapy and a Bachelor of Science in Zoology from Iowa State University, Ames, Iowa. Jason is certified in Isernhagen Work Systems’ Work Injury Management Programs, is a certified occupational hearing conservationist and is also certified in DOT drug collection and testing. He is a member of American Physical Therapy Association. He also provides sports therapy to Nevada High School and Colo-NESCO High School athletics.

Kevin Bultena, MSPT
Kevin is a Physical Therapist and the Clinic Director of Therapy Services at Hamilton Hospital. He is a graduate of Washington University, St. Louis, MO with a Master’s in Physical Therapy and a Bachelor of Science in Exercise Science from Iowa State University, Ames, Iowa. Kevin is a member of the Webster City Rotary Club and the Asbury Methodist Church.

Lindsay Burrhen, DPT
Lindsay, DPT, is a physical therapist.  She provides physical therapy services at Story County Medical Center in Nevada.  Lindsay is a graduate of the University of Iowa with a Doctorate of Physical Therapy degree.  She received her Bachelor of Science in Exercise Sports Science from Iowa State University.  Lindsay is a member of the American Physical Therapy Association.

Monica Clinesmith, DPT, LAT
Monica is a Physical Therapist and provides therapy services at Altoona and Indianola. She is a graduate of the University of Iowa with a Doctorate in Physical Therapy and a Bachelor of Arts in Biology with an emphasis in Athletic Training from Simpson College, Indianola, Iowa. She is a member of the American Physical Therapy Association.

Matt DeWall, MPT, CSCS
Matt is a Physical Therapist and Clinic Director of Indianola Physical Therapy. He is a graduate of University of Iowa with a Master’s in Physical Therapy and a Bachelor of Arts in Biology from Simpson College, Indianola, Iowa. Matt is certified in DOT drug collection and testing and is a member of American Physical Therapy Association (Orthopedic section), Iowa Physical Therapy Association, National Strength and Conditioning Association, and the Indianola Rotary Club. He also provides sports therapy to Indianola High School and SE Warren High School athletics.

Dana Hendrick, ATC, LAT
Dana is a certified and licensed athletic trainer.  She is a graduate of Simpson College with a Bachelor of Arts in Athletic Training.  She is a member of the National Athletic Trainer’s Association.  This past spring Dana was assistant girl’s soccer coach at Colo – NESCO High School.  Dana is the marketing director at 21st Century’s corporate office and provides sports medicine services to Bondurant – Farrar High School.

Lori Kibby, ATC, COTA
Lori is a licensed Athletic Trainer and Occupational Therapist Assistant. Lori is a graduate of Iowa State University with a Bachelor of Science in Exercise Science with a minor in Health Studies and Athletic Training and has an Associate of Applied Science degree in the Occupational Therapy Assistant program from Kirkwood Community College, Cedar Rapids, IA. She is a member of the National Athletic Trainers’ Association and American Occupational Therapy Association. Lori also provides sports medicine services to Perry High School athletics.

Chad Kuehl, MPT
Chad is a Physical Therapist at Dallas County Hospital. Chad is a graduate of Des Moines University with a Master’s in Physical Therapy and Bachelor of Science from Northwest Missouri State University, Maryville, MO. He is a member of the American Physical Therapy Association and Iowa Physical Therapy Association.

Clint Lutterman, DPT
Clint is a Physical Therapist and the Clinic Director of Therapy Services at Dallas County Hospital.  He is a graduate of Creighton University of Omaha, NE with a Doctorate in Physical Therapy and a Bachelor of Arts in Biology from Northwestern College in Orange City, IA. He is member of American Physical Therapy Association (Sports Physical Therapy section) and the Perry Rotary Club. Clint provides sports therapy to Perry High School athletics.

Amanda Marine, MA, SLP
Amanda, SLP, is a speech-language pathologist.  She provides speech therapy services at Story County Medical Center in Nevada, Dallas County Hospital in Perry and Hamilton Hospital in Webster City.  Amanda is a graduate of the University of Northern Iowa with a Masters of Speech-Language Pathology degree.  She received her Bachelor’s in Communicative Disorders from UNI as well.

Tamara Mechaelsen, PT
Tammy is a Physical Therapist at Hamilton Hospital in Webster City. She is a graduate of University of Minnesota with a Bachelor of Science in Physical Therapy. She is a member of the American Physical Therapy Association.

Emily Obrecht, PTA
Emily is a Physical Therapist Assistant at Story County Medical Center.  She is a graduate of Iowa Central Community College in Fort Dodge, IA with an Associate of Arts degree and has an Associate of Applied Science Degree in the Physical Therapy Assistant Program from Kirkwood Community College in Cedar Rapids, IA.  Emily is a member of the American Physical Therapy Association and St. Gabriel’s Catholic Church in Zearing.

Steve also provides sports therapy to Bondurant-Farrar High School athletics.

Tara Phares, COTA
Tara is a certified occupational therapy assistant at Story County Medical Center.  Tara is a graduate of Kirkwood Community College in Cedar Rapids, IA with an Associates of Applied Science Degree in the Occupational Therapy Assistant Program.  She is also a member of the American Occupational Therapy Association, Brain Injury Association of Iowa, National Multiple Sclerosis Society and Brain Injury Association of America.  Tara also provides occupational therapy services to Hamilton Hospital in Webster City and Altoona on an as needed basis. 

Kerri Pedersen, ATC, LAT
Kerri is a certified and licensed athletic trainer.  She is a graduate of the University of Northern Iowa with a Bachelor of Arts in Athletic Training.  Currently Kerri is also working on her Master’s in Kinesiology with an emphasis in Health Promotion and Exercise at Iowa State University.  Kerri is a member of the National Athletic Trainer’s Association.  She is also involved with Relay for Life through the American Cancer Society.  Kerri works at 21st Century Rehab’s Indianola location and provides athletic training services to Indianola High School.

Tracy Porter, MSPT
Tracy is a Physical Therapist and Clinic Director of Madrid. She also provides therapy services at Story County Medical Center, Rolling Green Village and Kryslis in Nevada, as well as home health services. Tracy is a graduate of Des Moines University with a Master’s in Physical Therapy and a Bachelor of Arts in Biology from University of Northern Iowa, Cedar Falls, Iowa. She is a group fitness instructor for Ames Racquet & Fitness and is a member of Memorial Lutheran Church in Ames where she participates in choir and is a Sunday School teacher. Tracy is involved with One Heart Equestrian Therapy and is a member of Beta Sigma Phi.

Jamie Rabe, DPT, ATC       
Jamie, DPT, is a physical therapist and certified athletic trainer.  She provides physical therapy services at Story County Medical Center in Nevada as well as the Madrid location.  Jamie is a graduate of the University of Iowa with a Doctorate of Physical Therapy degree.  She received her Bachelor of Science Degree from Iowa State University.  Jamie is a member of the Collegiate United Methodist Church and also an Iowa Special Olympics Volunteer.  

Kate Reiter, MAOT, OTR/L
Kate is an Occupational Therapist at Dallas County Hospital in Perry. Kate also provides occupational therapy services to a number of the 21st Century Rehab clinics including the Altoona, Grimes, and Indianola clinics and Story County Medical Center in Nevada. Kate is a graduate of the College of St. Catherine, St. Paul, MN, with a Master’s degree in Occupational Therapy and a Bachelor of Science degree from Iowa State University in Exercise and Sport Science with a minor in Health Studies.

Nancy Hunter Schreier, OTR/L
Nancy is an Occupational Therapist at Story County Medical Center. She is a graduate of Iowa State University with a Bachelor of Science in Psychology and Bachelor of Science in Occupational Therapy from College of St. Catherine, St. Paul, MNNancy is a member of the American Occupational Therapy Association and St. Patrick’s Catholic Church.

Jodie Stokke, MS, CCC/SLP
Jodie is a Speech Pathologist for 21st Century Rehab. She is a gradate of Southern Illinois University, Carbondale, IL with a Master’s in Communication Disorders and Sciences and a Bachelor of Science in Psychology and Sociology from Iowa State University.  Jodie is a member of American Speech-Language Hearing Association, Iowa Speech-Language Hearing Association and Harvest Evangelical Free Church.  Jodie is involved in Meals on Wheels, AWANA and a Roland-Story school volunteer.

Marc Vander Velden, MPT, CSCS
Marc is a Physical Therapist and Clinic Director of Grimes. Marc is a graduate of University of Iowa with a Master’s in Physical Therapy and a Bachelor of Arts in Biology from Central Iowa, Pella, Iowa. He is certified in Isernhagen Work Systems’ Work Injury Management Programs and DOT drug collection and testing. He is a member of American Physical Therapy Association, Iowa Physical Therapy Association, the Grimes Chamber of Commerce, National Strength and Conditioning Association, and the Grimes Jaycees. Marc also provides sports therapy to Woodward-Granger High School, Dallas Center-Grimes High School and Des Moines Christian athletics.

Tasha Vorm, ATC, LAT
Tasha is a certified and licensed Athletic Trainer at Story County Medical Center. Tasha is a graduate of Iowa State University with a Bachelor of Science in Exercise and Sport Science with a minor in Athletic Training. Tasha is a member of National Athletic Training Association and the Iowa Athletic Trainers' Society, head softball coach for Colo-NESCO High School, a second grade religion teacher at St. Patrick’s Catholic Church and is the secretary for St. Patrick’s Life and Growth Committee. Tasha also provides sports medicine services for Nevada High School and Colo-NESCO High School athletics.

Curtis Waddingham, ATC, LAT
Curtis is a certified and licensed athletic trainer and is the Director of Sports Medicine Services and Promotions. He is a graduate of Simpson College with a Bachelor of Exercise Science and minor in Biology. Curtis is also a licensed Radiologic Technologist for Iowa Methodist Hospital where he works part-time. Curtis is a member of the National Athletic Training Association and the Simpson College Athletic Booster Club. Curtis provides sports medicine services to Dallas Center-Grimes High School and Woodward-Granger High School athletics.   

Teresa White, PTA
Teresa is a Physical Therapist Assistant at Dallas County Hospital. She is a graduate of Indian Hills Community College, Ottumwa, IA with an Associate in Applied Science in the Physical Therapist Assistant program. She serves on the Dallas County Hospital Public Relations Committee (P.R.I.D.E.) and C.A.R.E. Subcommittee, is part of Dallas County Hospital Volunteer Auxiliary, Ladies Nite Out (provides women's health education seminars), and Dallas County Hospital Standards Committee.

Sara Zorn, COTA/L
Sara is a certified occupational therapist assistant at Hamilton Hospital and Story County Medical Center.  She is a graduate of Western Iowa Tech Community College, Sioux City, Iowa with a degree in Occupational Therapy.  Sara is currently pursing a degree in Occupational Therapy. Outside of work, Sarah is a volunteer EMT with the Williams Ambulance Service and also enjoys spending time with her family.
