21st Century Rehab, P.C. provides physical, occupational, and speech therapy, sports medicine services, and occupational health services. 21st Century Rehab is certified in pre-work screening, job matching systems, functional capacity evaluations and stay-at-work/return-to-work program known as WorkWell, designed by Isernhagen Work Systems.
We also provide therapy services and consultation to skilled nursing facilities including Rolling Green Village in Nevada, Perry Lutheran Home and the Masonic Home in Perry. We consult and provide services for several private and public home health agencies.
21st Century Rehab was founded in 1995 by Steve and Michelle Cassabaum and has five outpatient clinics locations in Altoona, Carlisle, Grimes, Indianola and Madrid; and four hospital locations at Dallas County Hospital (Perry), Franklin General Hospital (Hampton), Hamilton Hospital (Webster City) and Story County Medical Center (Nevada). 21st Century Rehab is headquartered in Nevada.
